Hi Erik,
I think a good application portfolio would focus on demonstrating these three qualities.
1) Your skills
Basically you want to show what you can do. You're not expected to be an all-star at this stage but you should focus on demonstrating your fundamental abilities. A way of doing this is by sketching existing products. Maybe you've taken art in high school, and have some stand out projects that are impressive?
2) Your creativity
You're going to want to show how you think, and demonstrate the potential you have to be a successful designer. The ideas that you're jotting down in your sketch book could be an interesting way of achieving this, but you could supplement that with other pieces that have an emphasis on displaying your creativity.
3) What you are passionate about and what makes you unique?
Everyone has passion projects, or particular interests that are unique to them. Aside from your skills, and thought process, what makes you who you are? Do you have any personal projects you've undertaken that you just did for fun? This kind of thing will give you a chance to put your personal touch on your application.
Hope this helped Erik. Be sure not to forget to put time and care into the presentation of your portfolio itself, not just the content within! If you can incorporate some of the points you want to get across into your actual portfolio presentation, that's gotta be some bonus points...
Good luck!

Thank you very much Michael for the in-depth review. I have been searching for months to find a good review like this. I am in grade 12 at the moment and I am working my hardest to build a solid portfolio for the application along with trying to keep my grades up. If you would be able to help me out, I am trying to figure out what the emphasis' of my portfolio should be. At the moment I am focusing heavily on ideas (jotted down in my sketch book with multiple sketches), perspective drawings/investigations and 3-D pieces (mostly sculptures). Is there anything else, off the top of your head, that I should spend a lot of time focusing on in my portfolio?
Thank you for your time,
